Illness / Medical Emergency
Employees who are absent for three or more consecutive workdays due to an illness may be asked to provide certification from their health care provider that they are fit to return to work with or without reasonable accommodation to perform the essential functions of their job. If the employee has a medical emergency and requires a visit to a medical facility and is unable to work or complete a shift, certification of the visit and of the employee’s ability to return to work with or without reasonable accommodation from their health care provider must be faxed to us at 317-660-8703 or emailed to us at support@comforcare.zohodesk.com.
