Incident Reports
Employees who witness, become aware of, receive information about, or are involved in any incident, unusual occurrence or happening that is not consistent with any of the Company’s policies and procedures, whether published or unpublished, must immediately call their Supervisor or any Manager and complete an incident report. The written incident report must be promptly completed and received at the office no later than 24 hours after the incident. Any report of such an incident will be treated with as much confidentiality as possible in each case. However, the Company’s investigation of an incident report may require interviews of other individuals regarding allegations about the incident. In order to promote thorough communication between field staff and office staff, as well as high quality care for all clients, all employees are encouraged to report (verbally and in writing) any such occurrences even if the employee is not sure the circumstances warrant such a report. Examples of incident reports include falls; medication errors; hospitalizations; suspected or actual assault, abuse, neglect, exploitation or mistreatment of a client; death of a client; and anything else that could potentially impact the health or safety of a client or the ability to provide appropriate care for the client. Cooperation with an investigation of an incident report is mandatory. Employees who refuse to cooperate or who are not truthful during an investigation may be subject to disciplinary action up to and including termination of employment. The Company prohibits retaliation against any employee who reports in good faith an incident or who participates in good faith in an incident investigation. If you believe you are being retaliated against for your involvement with incident reporting or investigation, you should report this violation of Company policy to any one of the following management members: your Supervisor; any Manager; or the Owner. Supervisors and Managers must report violations of this policy immediately to the Owner.
