Documentation
Documentation (and travel logs if applicable) is to be completed on a daily basis for each shift. Your documentation must reflect the actual hours that you have worked and must be signed by you and by the client. You have two options for completing your documentation. Option 1 is to correctly complete your documentation through the caregiver app. Once completed, it is immediately sent to us, you have no further responsibility. Option 2 is to complete your documentation through our telephony system. In emergency situations (situations where the caregiver app and telephony system are not working), you may complete a hard copy flow sheet. Hard copy flow sheets are due by Tuesday at noon for the previous work week (Saturday – Friday). Travel logs can be completed electronically through Google Forms, scanned and emailed to support@comforcare.zohodesk.com, or mailed/dropped off at the office. Travel logs are due by noon on Tuesday for the previous work week (Saturday – Friday). Travel logs not submitted within two weeks of the shift will not be paid. If you do not complete your documentation through the caregiver app or the telephony system, you will be required to come into the office to complete it.
If the documentation (and travel logs if applicable) is not filled out correctly, missing, or late, you will have to come into the office to correct it and/or receive disciplinary action. You will also not be eligible to receive your QPB for the payroll period in which you have incorrect/late/missing documentation for that payroll period. You will also be issued a live check that can be picked up at the office. Again, you will face disciplinary action up to and including termination.
Payroll Late Documentation
Documentation should be completed daily for each shift worked. Any documentation not submitted daily will be considered late and will result in disciplinary action. You will also not be eligible to receive your QPB for non-overtime hours worked in the payroll period that you have late documentation.
Missing Documentation
Any documentation (and/or travel logs if applicable) not submitted daily, and found to be missing by the administrative staff, will be considered missing documentation. Missing documentation will result in disciplinary action. You will also not be eligible to receive your QPB for non-overtime hours worked in the payroll period that you have missing documentation.
Incorrect Documentation
If mistakes are found on documentation (and/or travel log if applicable), it will be considered incorrect documentation. These mistakes may include dates and days of weeks that do not correspond, time in and time out that do not correspond with the total hours, missing signatures and missing names. Turning in incorrect documentation will result in disciplinary action up to and including termination. You will also not be eligible to receive your QPB for non-overtime hours worked in the payroll period that you have incorrect documentation.
