Every employee starts out earning an hourly base pay rate of $10.00 for client services. Employees are eligible to receive a Quality Performance Bonus (QPB) premium paid on top of this hourly base pay rate if the employee completes All OF THE REQUIREMENTS LISTED BELOW WITHOUT EXCEPTION in accordance with this policy.
The QPB premiums do not apply to client services performed in overtime hours. In other words, the QPB premiums only apply to client service hours that are worked within a 40-hour workweek within the bonus period. Your eligibility for QPB premiums (i.e., the bonus period) is every two work weeks of your payroll period. So, if you do not meet the QPB requirements described in this policy for any workday that falls in one payroll period, you may still be eligible to earn QPB premiums in subsequent payroll periods.

Mileage Reimbursement Rate: $ .50 / mile
Again, the hourly QPB premium rate is paid as a bonus UPON COMPLETION OF ALL REQUIREMENTS listed below in this policy without exception. FAILURE TO MEET ANY ONE OF THE REQUIREMENTS WILL RESULT IN THE LOSS OF YOUR QPB ELIGIBILITY FOR THAT ENTIRE TWO-WEEK PAYROLL PERIOD AND YOU WILL BE PAID AT YOUR HOURLY BASE PAY RATE FOR CLIENT SERVICES.
● No unexcused call-offs*, late arrivals, or early departures unless the absence is protected by law.
○ *An unexcused call off is when you call off for a shift when it is not an emergency situation or if you call off for an emergency situation but fail to provide documentation within 72 hours of the emergency situation unless the absence is protected by law and you provide documentation as soon as reasonably possible.
● No unexcused no call / no shows* unless the absence is protected by law.
○ *No Call / No Show is defined as failure to appear for any scheduled shift without properly notifying the office at least 4 hours in advance of the shift start time. A no call / no show may result in immediate termination unless the absence is protected by law.
● Submitting incorrect/late/missing documentation for your time worked (see “Documentation and Records,” and all other applicable sections on pages 36-38, below).
● If you resign without providing and working a two-week resignation notice period, you will not be eligible to receive your QPB in the payroll period your resignation date occurs, unless a shorter period is required by the Company or your absence during the resignation period is protected by law.
● If you are terminated for cause (i.e., violating a Company policy or procedure or poor work performance), you will not be eligible for your QPB in the payroll period your termination date occurs.
* Your hourly QPB (bonus) premium rate of pay will be based on your education, your professional caregiving experience, and your professionalism during the interview and orientation process.
