Physician Orders For Scope Of Treatment (POST)


In consultation with a legal representative, a physician will write orders for a client that reflects his/her wishes with regards to cardiopulmonary resuscitation (CPR), medical interventions (comfort measures, limited additional interventions, or full treatment), antibiotics and artificially administered nutrition.  Additionally, the client has the option on the POST form to designate a Health Care Representative.  

 

Executed POST forms may be revoked at any time.
